i think your medals are in relation to your solo+party mmr as compared to other players in your bracket. So its a percentile system, so far as i can make out.
Had the same experience. 3750 to 3950 and grew from legend 3 to ancient. I got legend 0 on 3.5k or something like this.
But I completely changed my playstyle, moved to support and got pretty decent winrates on them.
I guess medals depends on your skill in the matches and if you have a streak of good games, which you dominate, you move up.
Even if your mmr didn't improved probably you had good games compared to others.

i calibrated at 1400 mmr then went upto reach 1900 and earned crusader2. Then went down to around 1700 mmr and stayed there for a while and when i went back to 1900 with a winning streak i became crusader 3 then 2000 crusader 4 . Then again lost and went back to 1900 then reached 2k again and became crusader 5. How can they have different medals for same mmr?
